Every Org accumulates patterns. A trigger framework from 2019. Another one from 2022 because the second team didn't like the first one. Error logging that works one way in Service Cloud and another way in Sales Cloud. None of these are wrong on their own. The problem is having more than one.
The cost is cognitive load. Every developer who opens a class has to ask which pattern this one follows. That tax gets paid daily, by everyone, forever until somebody unifies it.
Most teams skip the hard part. They build the new framework, roll it out for new development, and call it done. The old occurrences stay untouched. Now there are two patterns instead of one, plus a new rule about which applies when.
Real cleanup means going back and bringing every old Trigger, every legacy handler, in line with the new standard. Less visible but it's the difference between an Org that's actually easier to work in and one that just has more patterns to choose from.
