What I keep finding.
Notes from five audits in one week, combined across Orgs. No single Org looks like this, but every Org had some of it.
User interfaces are the biggest issue. Account is the dirtiest Object, even in Orgs that keep Case and Opportunity clean.

What do I need to prepare?
Nothing. No slide decks, no pre-reads. Just bring screen-share access to a Sandbox, and invite the Org Owner or someone who knows the history of the Org.

What do I get afterwards?
